How to get your records for a second opinion

Start with a patient copy

Ask medical records or the patient portal for a patient copy of the pathology report, recent imaging, and the latest clinic note. A transfer to another office is useful, but a copy you can hold helps every later step.

You do not need a diagnosis recap to make this request. Names, dates, and the facility are enough to start.

If the process feels heavy

OncoKind Concierge can collect records and submit the packet to the center you choose after you authorize it. We do not interpret results. Hospitals set their own timelines.
